ATTENTION PARENTS AND STUDENTS:

 

All following issues of the Upward Bound Newsletter will include the absence form below.  In the event a participant will need to be absent from a scheduled Upward Bound Meeting, she/he must complete this form and return it to Jami Nichols within seven (7) days of the missed meeting.  If the absence is due to a school related activity, the form must be signed by a school official.  You may make copies of this form.
